Before you start

Check the client's name and your record-access permissions. You are responsible for the content of your own notes and care plans.

Follow these steps

  1. 1. Open the client

Choose Clients and open the intended client's workspace.

    Open clients
  2. 2. Open Notes or Care Plan

Choose Notes and select Create note, or choose Care Plan and select Create care plan. Open an existing entry if you want to review or update it.

  3. 3. Write and review your entry

Enter your own note or care plan. Review the client, date, title, and content before saving. Keep public-profile writing and ordinary inquiry email separate from private records.

  4. 4. Save and verify

Select Save draft and wait for confirmation. Reopen the entry to check it saved. When a note is ready to finalize, select Finalize note and review before selecting Confirm. Later note corrections use Add amendment, then Save amendment.

What happens next

The entry remains in the private client record with its revision history. It is not automatically shared with the client, referral partners, or an affiliated organization.

If you get stuck

My changes are not saved.
Read any error on the page and keep the editor open while resolving it. Use the save confirmation and reopened entry to verify your work before leaving.
How do I share something with the client?
Use a form, message, or file intended for sharing. For a care plan, review the version and use Share this version when offered. Private notes are not automatically visible to the client.
